Findings: Witness 1 (W1) has a known history of resident-to-resident altercations. On or about July 10, 2021, AV was in the courtyard with a relative when W1 was bothering them. AV then moved to a different location in the courtyard when W1 tried to pull AV’s wheelchair. AV told W1 to leave AV alone which W1 responded by punching AV in the face. AV sustained a swollen nose due to the injury. The facility failed to provide a safe environment to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-00921 $338.00 fine assessed
