Findings: On July 13, 2021, the facility failed to provide a safe environment for Alleged Victim (AV) which resulted in a resident-to-resident altercation between AV and Witness 1 (W1) in which W1 called AV a derogatory name which caused AV emotional harm. An investigation determined there has been a history of issues and incidents between AV and W1 and facility failed to implement interventions to prevent future altercations. The facility's failure is a violation or resident rights, is considered neglect of care and constitutes abuse.
