Findings: The facility failed to implement interventions and appropriately care plan after the Alleged Victim (AV) experienced a fall on the morning of June 10, 2021. AV experienced another fall in the afternoon, which was not documented, and was sent to the hospital to rule out a possible head injury. The facility's failure to document and communicate proper care instructions after both falls, is a violation of resident rights, is considered neglect of care and constitutes abuse.
