- Findings
- Alleged Victim (AV) is care planned to receive two full-assist showers weekly. On or about June 16, 2021, it was reported that AV had gone without a shower for eight days. The facility failed to provide showers resulting in AV experiencing skin breakdown which is a violation of resident rights, is neglect of care which constitutes abuse.
