Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about July 3, 2021, AV reported his/her wallet went missing from his/her apartment. Alleged Perpetrator 2 (AP2) was on shift the day AV’s belonging went missing as well as the other shifts when staff reported thefts from the break room. Facility video recording shows theft from the facility by AP2. AP2 failed to protect AV from financial exploitation, which is neglect of care and constitutes abuse. The facility failed to protect AV from financial exploitation, which is violation of resident rights, is neglect of care and constitutes abuse.
