Findings: On or about July 4, 2021, a bruise was discovered on the Alleged Victim's (AV) hip. AV has had multiple falls, although his/her care plan notes that AV is independent in transferring. On or about July 15, 2021, AV was found on the floor next to his/her bed. AV has suffered a broken tooth, although it is unknown when it happened. The facility failed to properly care plan to ensure AV's safety from falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
