Findings: On or about July 16, 2021, Alleged Perpetrator 2 (AP2) was being trained on medication administration. AP2's trainer/observer became distracted with another resident during the training and asked AP2 to pause administration. AP2 instead continued administration of medication and mistakenly gave Alleged Victim (AV) another resident's medication cup. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe medication administration which is a violation of Oregon Administrative Rules.
