Findings: Alleged Victim (AV) relies on the facility for h/h care. AV has a history of bruises from an unknown cause. On or about July 17, 2021, AV was found to have multiple bruises on AV's forearm and one on the chest. The facility failed to appropriately care plan to mitigate the risk of bruising from an unknown cause, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
