Findings: On or about July 18. 2021, the Alleged Victim (AV) was found outside, down on the ground, laying in a bush. AV suffered multiple pin prick sized skin tears on his/her left arm. AV is a known fall risk and needs assistance with ambulation. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
