Findings: Alleged Victim (AV) relies on the facility for his/her care. AV and Witness 3 (W3) have a history of resident-to-resident negative interactions with each other. On or about July 16, 2021, W3 was yelling and berating AV. The facility failed to redirect W3 and AV away from each other until AV became emotionally upset, which is a violation of resident rights, is considered neglect of care and constitutes abuse. Alleged Preparator 2 (AP2) was present when W3 berated AV. There are multiple conflicts with witness and documented statements showing AP2 allegedly failed to protect Alleged Victim from verbal abuse. An investigation determined no AP2 wrongdoing or abuse occurred.
