Findings: On or about July 19, 2021, the Alleged Victim (AV) suffered two falls. In the first fall, AV suffered multiple injuries and was assisted back to bed. AV fell again, later that same day while in the shower. AV had significant bruising from both falls. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01562 $375.00 fine assessed
