Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about July 18, 2021, staff noticed a dark purple bruise and a cut below AV’s left elbow. On or about July 19, 2021, AV went into another resident’s room to use the bathroom and fell; AV was assisted up and able to walk with his/her walker and no assistance. On or about September 13, 2021, while changing AV’s clothing, staff noticed a large bruise about 8-9cm on his/her right arm and skin tear on top of that bruise. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
