Findings: Alleged Victim (AV) relies on the facility for a safe environment. On or about July 20, 2021 AV and Witness #1 (W1) were in a resident-to-resident altercation. W1 has history of prior resident-to-resident alterations, and his/her service plan was not updated to address the behaviors. The facility failed to provide a safe environment and failed to properly care plan which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03301 $375.00 fine assessed
