Findings: Alleged Victim (AV) has a known history of falls. Between May 24th, through July 20, 2021, AV sustained approximately nineteen (19) falls, some were witnessed, some falls resulted in injury. On or about July 15, 2021, AV had an injury fall resulting in a scrape on AV’s shoulder. The facility failed to properly plan care appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-00899 $1500.00 fine assessed
