Findings: The Alleged Victim (AV) was care planned to have safety checks every two hours. On or about July 20, 2021, AV fell out of bed during the night or early morning and was potentially on the ground from 4:00 am to 8:00 am. AV suffered a bruise to his/her hip. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
