Findings: The facility failed to provide a safe environment for Alleged Victim (AV) during the time period of May 7, 2021 through July 16, 2021 which resulted in AV suffering eighteen reported falls inside the facility with seven of these falls resulting in injury to AV with two of the seven injury falls resulting in head lacerations requiring treatment with staples at emergency department. An investigation determined that fall interventions were unchanged from previous interventions and were not person-specific, but generic in nature, and did not address the fact that many of AV's falls were unwitnessed. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00162 $1500.00 fine assessed
