Findings: Witness 1 (W1) has had prior resident-to-resident altercation. On or about July 24, 2021, Alleged Victim (AV) was looking out the dining room window at a spot that W1 chooses to sit at. W1 self-propelled over to the table where AV was and W1 used his/her wheelchair to attempt to push AV out of the way. The facility failed to provide a safe environment for AV, which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: ALFCP22-00644 $188.00 fine assessed
