Findings: Alleged Victim (AV) is a care planned to have 24-hour supervision due to high elopement risk. On or about July 26, 2021, staff received a call from a sister facility stating that AV was in their building. AV was in the courtyard and was able to open the gate and walk out. The facility failed to follow the care plan around AV’s elopement risk, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-03400 $500.00 fine assessed
