Findings: Witness 1 (W1) has a history of physical altercations. On or about July 25, 2021, W1 was showing behaviors toward Alleged Victim (AV) because AV moved into W1’s room. On or about July 26, 2021, W1 believed AV was going through his/her closet. W1 was afraid AV was going to take W1’s clothes and warned AV to get out of W1’s closet. AV and W1 yelled back and forth at each other and W1 punched AV in the face resulting in AV falling and sustaining an injury. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-00521 $375.00 fine assessed
