- Findings
- The Alleged Victim (AV) had approximately seven (7) falls in less than sixty (60) days, from which AV was diagnosed with abrasions to his/her head and fractures to his/her vertebrae. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
