Findings: Alleged Victim (AV) relies on the facility for his/her care and a safe environment. AV and Witness #1 (W1) have a history of verbal and physical altercations. On or about June 29, 2021 AV and W1 had a verbal and physical altercation resulting in W1 having a scratch on W1’s cheek. The facility failed to care plan interventions to attempt to prevent resident-on-resident altercations for AV and W1.
