Findings: Witness 1 (W1) is care planned to be kept away from Alleged Victim (AV) due to prior Resident-to-Resident altercation. On or about July 29, 2021, AV was trying to keep W1 out of other rooms when W1 begun to smack AV in the face. AV had a red mark on his/her face. The facility failed to follow W1’s care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
