Findings: On or about July 29, 2021, Alleged Victim (AV) was found by facility staff sitting on his/her bed bleeding from his/her toes with a significant amount of blood observed around him/her. An investigation determined that facility staff had been aware of AV's bleeding for approximately an hour and a half before action was taken to address AV's injuries. It was determined that unclear communication between facility staff contributed to the staff's delayed response to AV's injury. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03660 $225.00 fine assessed
