Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about July 30, 2021, shortly after lunch was served, AV was found “slumped over” in bed, wearing nothing except a brief; AV and the bed were soaked in urine and covered with food. AV appeared to have dropped a cheeseburger in his/her bed and had dried vomit on him/her. There appeared to be food from other meals all over AV and the bed, the entire bed had to be wiped down and waterproof pads placed under the sheets. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
