Findings: On or about July 30, 2021, Alleged Victim (AV) was discovered on the floor of his/her bedroom around 4:30am. An investigation determined that AV pulled his/her call light on 7/29/2021, between 8:30pm and 8:45pm and waited in his/her wheelchair with no assistance from facility staff. As AV waited, he/she became fearful and frightened, and then attempted a self-transfer resulting in the fall. It was later determined that AV's call light was not working properly at time of incident. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-03316 $225.00 fine assessed
