Findings: Witness #1 (W1) is known to have behaviors and had been having behaviors the day before and earlier the day of the incident. The Alleged Victim (AV) was standing in the TV area watching TV when W1 got mad and pushed AV out of the way. AV was not injured in the incident, however, AV was placed at risk for harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
