Findings: Alleged Victim (AV) is care planned to be checked four (4) times on each shift. AV reported attempt to self-transfer on or about August 04, 2021, at approximately 10:00pm and sat on the floor. On or about August 05, 2021, at approximately 8:25am, AV was found lying on the floor. Camera review shows AV was not checked on during the night. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
Sanction: ALFCP22-00517 $188.00 fine assessed
