Findings: On or about July 6, 2021, the Alleged Victim (AV) was assessed as having a significant change of condition, requiring increased needs in toileting. AV's care plan was not updated to reflect the needed changes to his/her care. On or about August 7, 2021, AV was found on the floor of his/her room, had a soaked brief, wearing no footwear and had an injury to his/her head. AV was sent to the hospital and was found to have a pelvic fracture. The facility failed to properly care plan for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
