Findings: Alleged Victim (AV) relies on facility staff to ensure resident’s rights. From July 16, 2021 to August 8, 2021, AV and Witness 1 (W1) had four altercations. W1 was care planned to have one on one (1:1) caregiving at the time of the August 8, 2021 incident. Alleged Perpetrator 2 (AP2) did not follow W1's care plan to ensure a safe environment for AV. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
