Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about August 9, 2021, at approximately 8:10pm, facility staff were looking for AV to administer his/her medications and could not locate AV. AV was last seen in his/her apartment approximately 5 minutes prior. AV was located outside the secured unit in the parking lot unattended. The facility failed to provide AV with a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
