- Findings
- The Alleged Victim (AV) was a known exit seeker. On or about August 10, 2021, the AV eloped from the facility through a gate that had been left unlatched the previous day by landscapers. AV was found walking up a freeway off ramp. AV had an abrasion on his/her right arm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
