Findings: On or about April 21, 2021, the Alleged Victim (AV) was walking to his/her room with staff and lost his/her balance and fell backwards. AV was sent to the hospital for evaluation. AV received a bump on the head due to the fall. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
