Findings: The facility failed to follow the care plan. The Alleged Victim (AV) is care planned to have mechanically soft foods that are cut up. According to documentation, between July 6, 2021, and July 12, 2021, there are numerous instances where AV was given food that was not cut up resulting in AV aspirating, having difficulty swallowing and not eating. During the same time frame AV experienced numerous falls, and on or about July 13, 2021, AV fell and went to the hospital due to weakness, dehydration, and aspiration complications. These failures are a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00083 $500.00 fine assessed
