Findings: Alleged Victim (AV) relies on the facility for his/her care. AV had eight falls between April 4, 2021, and June 27, 2021. The facility failed to implement any new interventions after the fall on June 3, 2021, and on or about June 27, 2021, AV had another fall that resulted in a bruise. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
