Findings: On or about August 10, 2021, the facility failed to protect Alleged Victim (AV) from a resident-to-resident altercation with Witness 1 (W1). An investigation determined that AV wandered into W1's room in which W1 responded by punching AV in the back. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
