- Findings
- Alleged Victim (AV) relies on the facility for his/her care. On or about July 20, 2021, AV's doctor sent the facility a communication requesting AV's medication be placed on hold for an upcoming procedure. On or about July 26, 2021, AV was administered the medication leading to AVs procedure needing to be rescheduled. The facility failed to provide a safe medication administration system which is a violation or Oregon Administrative rules.
