Findings: The Alleged Victim (AV) has a history of falls and is care planned to have hourly safety checks. On or about August 12, 2021, AV was found on the floor of his/her room. AV did not suffer any injury, however, AV was placed at risk for serious harm, The facility failed to ensure AV's care plan was being followed to check AV every hour. The facility's failure is a violation of resident rights, is considered neglect of care an constitutes abuse.
