Findings: The Alleged Victim (AV) has had previous falls. On or about August 13, 2021, AV fell while walking from the bathroom in his/her apartment to a chair in his/her room. AV fell and complained of pain. The facility failed to properly care plan after earlier falls to ensure the safety of the resident. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
