Findings: The Alleged Victim (AV) is a known fall risk and has had 3 recent falls. On or about August 14, 2021, AV pushed his/her pendant, staff arrived at the room to find AV on the floor. AV was trying to get up to go to the bathroom and fell, resulting in a broken arm. The facility failed to properly care plan and place interventions in place to ensure the safety of AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
