Findings: The Alleged Victim (AV) is care planned to have toileting assistance every two hours and to have a visual check every hour for fall prevention. On or about August 17, 2021, at 3:34 am, AV was found on the floor of his/her room from a fall. An internal investigation determined no one had checked on AV as care planned. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
