Findings: The Alleged Victim (AV) is care planned to have toileting assistance and/or safety checks every two hours for fall prevention. On or about August 14, 2021, at 3:34 am, AV was found on the floor of his/her room from a fall. An internal investigation determined no one had checked on AV for approximately 3 hours. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-03493 $375.00 fine assessed
