Findings: On or about August 13, 2021, Alleged Perpetrator 2 (AP2) was witnessed yelling, threatening, and intimidating Alleged Victim (AV) while acting in the role of AV's caregiver which caused emotional harm to AV. AP2's actions constitutes verbal and emotional abuse. An investigation determined that the facility failed to keep AV's care plan up-to-date with AV's transfer care needs and to have that information available to staff. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP22-00234 $250.00 fine assessed
