Findings: The Alleged Victim (AV) was care planned to be a two person transfer at all times due to fragile skin and for safety. AV had multiple skin tears and an open wound on his/her buttocks from not being rotated. The facility failed to follow the care plan and to provide enough staff to meet the scheduled and unscheduled care needs of the residents. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
