Findings: The facility failed to implement interventions and appropriately care plan related to Witness 1's (W1) aggressive behaviors, in order to keep him/her and other residents safe. The facility's failure resulted in a resident-to-resident altercation between W1 and AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
