Findings: Alleged Victim (AV) relies on the facility for his/her care. AV and Witness 1 (W1) have history of aggressive behaviors and involvement in resident to resident altercations but not with each other. W1 had been showing signs of increasing behaviors starting on or about July 04, 2021. On or about August 22, 2021 AV and W1 were involved in a resident to resident altercation. The facility failed to properly plan care, and initiate interventions to respond to W1 increasing behaviors, which is a violation to resident rights, is considered neglect of care and constitutes abuse.
