- Findings
- On August 24, 2021, Alleged Victim (AV) eloped from the facility. An investigation determined that on day of the incident, yard maintenance had occurred and the courtyard gate was left propped open. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
