Findings: Alleged Victim (AV) relies on the facility for his/her care. AV requires constant supervision and extensive intervention. AV has history of wandering into other residents’ rooms. AV is care planned to have visual contact when AV is up and awake. On or about August 23, 2021, AV went into Witness 3 (W3) room and wheeled W3 out into the hallway were a resident-to-resident altercation took place. AV sustained a quarter sized skin tear to AV left arm. The facility failed to follow the care plan which is a violation of resident rights is neglect of care and constitutes abuse.
