Findings: The Alleged Victim (AV) was a known elopement risk and difficult to redirect. On or about August 23, 2021, AV exited the secure unit out the front door when a visitor opened the door and AV exited prior to the door closing. AV was found by staff outside near the road and was brought back to the facility. The facility failed to provide a safe environment, which placed AV at risk for harm. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
