Findings: Alleged Victim (AV) is a known wandering risk who is able to quickly ambulate without assistance. On or about August 24, 2021, AV was found alone in the facility parking lot by an agency staff member. The facility failed to prevent AV's elopement from Memory Care which is a violation of resident rights, is considered neglect of care and constitutes abuse.
