Findings: On or about April 2, 2021, the Alleged Victim (AV) and Witness #1 (W1) engaged in an altercation where W1 hit AV on the shoulder. There were no injuries, however, AV was placed at risk for harm. The facilities failure to properly care plan for W1's aggressive behavior, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
